Looking for a good medical doctor and diagnostician

Hi!  I also have difficulty absorbing vitamins and nutrients due to Crohn’s. Exhaustion and "brain fog" as you call it are strong indicators of low B12 levels.  I began having monthly injections about 1 year ago and it has done wonders for me.  The extra B12 has eleviated a good deal of joint pain as well. Your levels can be tested with a simple blood test.  Normal range is between 200 and 900.  Mine were just below 300.  My insurance wouldn’t cover the shots but a one year supply of needles and B12 costs less than $20.  Good luck! That would appear to rule out HIV, though it won’t hurt to get a test for it. Maybe a test for HTLV-1 as well?  That causes T-cell lymphoma, which usually affects the skin (mycosis fungoides) but can affect the gut too.  Very unpredictable in effects and severity, usually treatable.
